Controversies in Meniscal Injuries Treatment

Goal: Due to its functions in knee’s biomechanics, the menisci play a fundamental role in prevention of osteoarthritis in this joint. Thus, when they are damaged by some traumatic or degenerative patology, all joint is affected. This is a problem which Orthopedics community struggles with very often.

The objective of this study is to review theoretical concepts connected to this issue and also update some aspects of the menisci injuries’ treatment.
Material and Methods: The used was Pubmed, and it were selected only articles published in the last 10 years, with some exceptions that we considered justifiable because of its clinical relevance. The articles’ cientific quality was assured by the international prestige of the journal in which they were published.
Results: Whatever the treatment option taken, there will be changes in menisci function and so the cartilage will be exposed to major contact forces resulting sooner or later in pain and early osteoarthritic changes. In one hand some injuries have to be treated with total or partial resection, in the other a lot of different surgical techniques are being developed with the purpose of preserving the menisci by repairing and reconstructing them. The highlighted are allografts, implants, scaffolds, growth factor, tissue engineering and stem cells.
Conclusion: There are modern techniques for meniscal repair that seems to provide great and promising results. However further long-term studies are needed to answer some doubts about the future.
